The Recovery Timeline Nobody Tells You About
Hair transplant recovery is straightforward — but it has specific stages that every patient should understand before they fly to Turkey. Knowing what is normal versus what requires attention makes the entire process far less stressful.
This guide covers every stage from procedure day through to 12-month results, based on standard FUE and DHI protocols used by Istanbul's leading clinics.
Day 0: Procedure Day
Your procedure typically takes 6–9 hours including breaks. You will be awake throughout — local anaesthesia is used for both the donor and recipient areas.
What happens:
How you'll feel: Tired but not in pain. Mild pressure on the scalp. Mild swelling may begin to develop by evening.
Evening care:
Days 1–3: Immediate Post-Op
What to expect:
Day 1: Your clinic will perform the first wash. This is a gentle salt-water spray. Do not wash your hair yourself on day 1.
Days 2–3: Continue clinic washing routine if applicable, or follow the step-by-step wash instructions your clinic provides. Never rub — use only gentle patting motions.
What to avoid:
Days 4–7: Scab Formation
The small scabs (crusts) around each graft are completely normal. These are part of the healing process and protect the follicle as it anchors.
What to expect:
Washing routine (days 4–7):
Apply the foam provided by your clinic (or a gentle baby shampoo) and leave on for 40–45 minutes. Rinse with lukewarm water using low pressure — never hot water. Pat gently with a clean towel.
Flying home: Most patients fly home on day 2 or 3. Flying is safe from a medical standpoint. Cabin pressure does not affect grafts. Wear a loose surgical cap if needed. Stay well hydrated on the flight.
Week 2: Scabs Shedding
By the end of week 2, most scabs will have shed through washing. The recipient area will look cleaner and slightly pink.
Important: Some of the transplanted hairs will fall out along with the scabs. This is called shock loss and is completely normal. It is not a sign of transplant failure.
What to expect:
You can return to office work in week 2 if it is non-physical. Avoid gyms and strenuous activity.
Weeks 3–4: The Awkward Phase
The recipient area may look sparse as the transplanted hairs have shed. This is the most psychologically challenging phase — patients sometimes worry the procedure did not work.
What to expect:
The follicle is not gone. The hair shaft has shed but the follicle remains dormant below the skin, preparing to re-enter the growth phase.
Months 2–3: Dormant Phase
Very little visible change. The follicles are in the telogen (resting) phase. Some patients see very fine new hairs beginning to emerge by week 10–12.
Care at this stage:
Months 4–6: Growth Begins
This is when patients start to notice visible improvement. Fine, soft hairs emerge from the transplanted follicles and gradually thicken.
What to expect:
Month 6 is typically when patients can first see a clear result, though it is not the final result.
Months 7–12: Thickening and Maturation
The transplanted hairs thicken and mature. By month 9–10, most patients have approximately 80% of their final result visible.
| Months Post-Op | Approximate Result Visible |
|---|---|
| 1–3 | Dormant phase — little visible growth |
| 4–6 | Fine growth emerging |
| 6–9 | Clear improvement visible |
| 9–12 | 70–90% of final result |
| 12–18 | Full, mature result |
Month 12 photos are used by clinics for before/after comparisons — this is the gold standard comparison point.
When to Contact Your Clinic
Contact your clinic if you experience:
Normal: scabbing, mild itching, shock loss, swelling in the first week, temporary sparseness.
Follow-Up Consultations
Most reputable Istanbul clinics offer free 6-month and 12-month video follow-up consultations. Use these. Send progress photos. Your clinic can confirm whether your growth is on track and advise on any additional treatments (PRP, minoxidil) if needed.
